Sorry if you followed this link here from that old Writers in the Storm post! I no longer have the HTML template for you, because I lost it in a big web hosting move a year ago.
However, I will say this: I recommend using MailChimp for your mailing lists. They have a whole selection of really nice templates. The marketing information in the Writers in the Storm post still stands.
